Dr. Patel can correct a wide variety of so called "permanent" dental problems: • Missing teeth, gaps between teeth, general bite dysfunction • Chipped, cracked or worn teeth • Unsightly, stained or discolored teeth • Crooked or crowded teeth

Please arrive 10 minute prior to your appointment time, so that all paperwork may be completed before the time you are scheduled. Your initial appointment will consist of an exam and consultation which will include:

• Any necessary x-rays, photos, and models.
• An exam of your teeth and gums.
• A discussion about your concerns.
• A consultation and a treatment sequence.

*If there is a complex medical history or complex dental problem, it may require a more extensive evaluation and work up and a second consultation appointment.

IMPORTANT: All patients under 18 must be accompanied by a parent or guardian during the consultation visit.

Please alert the office if you have a medical condition that may be of concern (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are on any medication (i.e. heart medications, aspirin, anticoagulant therapy, etc.).

Your dental plan is a form of compensation provided by your employer. You can expect the carrier (insurance company) to reimburse you for a portion of our fee. That portion is determined by the contract between your employer and the insurance company. The higher the premium paid by your company, the more generous the reimbursement.

Although we are not a party to the contractual arrangement with your insurance company, we do want to help you receive the maximum reimbursement to which you are entitled. As a courtesy to you, we will help you process your insurance claims in order for you to receive this maximum benefit. We will also gladly provide dental x-rays and a written diagnostic report should your insurance company have any questions about the services provided.

Please remember that we are not contracted or affiliated with your insurance carrier. We do not work for any insurance companies. We work for you and your dental health only, therefore the responsibility of payment is yours whether or not you have a third party reimbursement.
